Creating a relapse prevention plan will be an important part of recovery from addiction. This worksheet provides a template for clients to list coping skills, social support, and the possible consequences of relapse. Additionally, there are several tips for your clients to refer to such as: "Cravings will eventually pass. Do your best to distract yourself and ride them out." and "The decision to relapse is made when you put yourself in risky situations, long before you actually use."

Ask your clients to keep their relapse prevention plan so they can refer to it during cravings or when they know they will be confronted with triggers.
